Admission Requirements
Basic Education Department (Preschool, Grade School, and Junior High School)
• Report Card – Original and Photocopy (must show 3rd Quarter Grades)
• Certificate of Good Moral Character
• Birth Certificate: Original and Photocopy
• Baptismal Certificate (if Catholic): Original and Photocopy
• Letters of Certification and Recommendation from the Principal and Guidance Counselor of the former school
• Four (4) pieces of recent 2”x2” studio photo (white background)
• Kindergarten Diploma or Certificate (for Grade 1 applicants)
• Elementary Diploma for Junior High School Applicants
Senior High School
• Original Grade 10 Report Card (F-138)
• Original F-137 (claim request letter from OLFU Admissions Office)
• Original or Certified True Copy of Certificate of Junior High School Completion
• Original Certificate of Good Moral Character issued in the current year
• Two (2) identical copies of recent 2”x2” studio photo (white background)
• Clear photocopy of PSA Birth Certificate
• Original Education Service Contracting (ESC) Certificate / Qualified Voucher Recipient (QVR) Certificate (if coming from private school)
College and Dentistry Freshmen
• Original Grade 12 Senior High School Report Card (F-138)
• Original F137 (with request letter from OLFU)
• Original Certificate of Good Moral Character issued in the current year.
• Two (2) identical copies of recent 2”x2” studio photo (white background)
• Clear photocopy of PSA Birth Certificate;
• For married female applicants, a clear photocopy of Marriage Certificate
• Clear photocopy of Grade 12 Senior High School Diploma
College of Medicine
• Must be a holder of any Baccalaureate degree
• Original College Transcript of Records
• Certified True Copy of College Diploma
• NMAT (National Medical Admissions Test) *required
• Photocopy of Birth Certificate
• Letter of Recommendation
• 2 Copies 2”x2” Picture
• Big Kraft Envelope (1 pc.)
Graduate School
• Original and one (1) photocopy of Transcript of Records with Special Order Number
• Honorable Dismissal
• Two (2) copies of 2”x2” colored identical photos
• Photocopy of PRC ID and board rating certificate (if licensed)
• Certificate of Employment
• Certificate of Good Moral Character by any of the following:
o School where bachelor’s degree was earned for those enrolling in the Master’s Program and school where MA was earned for those enrolling in the Doctor’s Program
o Certificate of employer (if currently employed)
Expanded Tertiary Education Equivalency and Accreditation Program (ETEEAP)
Pre-qualifications
• Must be a Filipino Citizen
• At least a high school graduate or the PEPT/ALS A and E result stating “qualified to enter in first-year college”
• Has at least five (5) years of work experience
• Proficient in the field of discipline that he/she is applying for
• At least twenty-five (25) years of age
• Accomplished ETEEAP application form
ETEEAP Admission Requirements:
• Letter of intent to avail of the program addressed to the ETEEAP Director
• Completely filled out ETEEAP Application Form
• Comprehensive Curriculum Vitae
• Academic Record – Form 137 or original Transcript of Records
• Honorable Dismissal
• Subject descriptions with learning competencies from the institution where the applicant last enrolled in.
• Service record or Certificate of Employment
• Duties and responsibilities in the various positions handled (signed by the HRD Manager or immediate unit manager)
• Latest 1″x1” ID Picture (5 pcs.) white background
• PSA Birth Certificate
• Barangay clearance or NBI clearance or Passport
• Other documents such as but not limited to:
o Certificate of participation and/or attendance in seminars, trainings,
conferences, colloquiums, or forums attended
o Recognition, awards, and certificates
o Relevant government examinations taken and passed
o Local and international certification
o Copy of research and/or articles published
o Certificate of professional membership
For Special Cases: Tranferees, Cross Enrollees, Alternative Learning System (ALS) Passers, International Students
Transferees
For evaluation of subjects that can be credited from your previous school, submit any one of the following documents:
• Scholastic Records
• Certificate of Grades
• Class Cards
• Transcript of Records
• Online Copy of Grades
Other Requirements:
• Honorable Dismissal / Transfer Credentials
• Transcript of Records (TOR) copy for OLFU
• Two (2) identical copies of recent 2”x2” studio photo (white background)
• Clear photocopy of PSA birth certificate
• For married female applicants, clear photocopy of the Marriage Certificate
Note: If you don’t have your requirements yet, submit a photocopy of your claim stub and receipt of Honorable Dismissal and TOR
Cross Enrollees
• Original Permit to Cross Enroll

Alternative Learning System (ALS) Passers
• Original Certificate of Rating
• Original Certificate of Good Moral Character
• Original or Certified True Copy of ALS Diploma

• Original Qualified Voucher Recipient (QVR) Certificate (if incoming Senior High School)
International Students
• Authenticated Transcript of Records
• Authenticated College Diploma
• Authenticated Letter of Financial Support (2Copies)
• Authenticated Police Clearance from Country of Origin
• Authenticated Bank Statement
• CEA (Certificate of Eligibility for Admissions)

IMPORTANT: All papers must be authenticated by the Philippine Embassy. OLFU processes study permits, and student visas upon acceptance into the school. OLFU offers special language programs to address the needs of foreign students.
