1. Application Form (download form - page 1 and page 2)
2. Original Form 138 - High School report Card or F137A
3. Original Certificate of Good Moral Character (From Principal or Guidance Counselor)
4. Clear Photocopy of Birth Certificate (present Original for Verification)
5. Clear Photocopy of High School Diploma (present Original for Verification)
6. 3 pcs 2" x 2" ID photos (White Background)
